The EOC requested that all schools distribute the annual parent survey to the parents or guardians of children in grades five, eight, and eleven, or in the highest grade in their school, excluding schools containing only grades two or lower. The survey instrument contains forty-six questions designed to elicit information on parental perceptions and parental involvement patterns.
