dc.description.abstract	This project was chosen to investigate the criteria, process, procedures and policies in place for transferring patients of the Charleston Dorchester Mental Health Center between programs due to a change in their level of care in efforts to make the process more consistent and prevent or reduce gaps in care for patients.
