Abstract
South Carolina Department of Health and Human Services, Community Long Term Care Division, administers the Community Choices Waiver to more than sixteen thousand medically and financially eligible South Carolina citizens. The Adult Day Health Care service is the most cost-effective service offered through the Community Choices waiver. To increase the number of participants who utilize the service of Adult Day Health Care from thirteen percent of the enrollment to eighteen percent of the enrollment was the goal of this project.
