Abstract
Healthy Connections Prime is an enhanced program that combines all of the benefits of Medicare and Healthy Connections Medicaid under a single Medicare-Medicaid Plan to make it easier for members to get needed health services. This update contains Advicare Advocate Claims ; HCBS Provider Phase II Transition FAQ Release ; Plans Receive Dementia Dialogues Training ; and 9th Annual National Fall Awareness Day.
